Im a single parent. My 12,yr old needs a passport?

I want to take my daughter to Jamaica in August. I have a passport, but she doesn't. Her father doesn't have contact with her and has been out of get life since she was 3. He is more than 17k in arrears in child support. He asked me not to contact him. Is there a way I can get her a passport for her without him?
